J&J Ottava Robot Wins FDA De Novo Authorization
Johnson & Johnson has received FDA de novo authorization for its Ottava robotic surgery system to perform multiple general surgery procedures, bringing the second heavyweight challenger to Intuitive Surgical’s da Vinci platform in the US. Announced on 22 July 2026, the authorization follows Medtronic’s Hugo clearance in December 2025 and clears the way for a controlled US launch with select customers as J&J targets additional indications over time.
Ottava is authorised for a broad set of upper-abdomen procedures, including Roux-en-Y gastric bypass, gastrectomy, cholecystectomy, splenectomy, gastric sleeve, small bowel resection, appendectomy, lysis of adhesions, fundoplication and hiatal hernia repair. J&J is positioning Ottava as the first soft tissue robotic system to integrate the robotic arms directly into the operating table, a hardware architecture materially different from the standalone cart-based designs used by Intuitive’s da Vinci and Medtronic’s Hugo. The application was supported by data from a pivotal Roux-en-Y gastric bypass study reported in May 2026.
Tim Schmid, J&J’s worldwide chairman for medtech, framed the launch as pioneering a new category of surgical robotics. Intuitive Surgical has dominated the US soft tissue robotics market for more than two decades, holding an estimated 80% share with da Vinci. Grand View Research valued the global surgical robots market at $6.6 billion in 2025, projecting growth to $18.5 billion by 2033. Alongside Medtronic’s Hugo and J&J’s Ottava, a wave of smaller entrants including CMR Surgical, Distalmotion and Moon Surgical are progressing US clearances, making the next two years arguably the most competitive stretch in the segment’s history.
The commercial signal is the effective end of Intuitive Surgical’s US monopoly on soft tissue robotics. With Ottava’s table-integrated design offering a genuine architectural differentiator and Hugo now competing on real-time AI, hospitals face their first meaningful robot choice in two decades. Expect intensified bundling, financing and training programme competition across all three players through the second half of 2026 and 2027.
