J&J Monarch Robot Gets FDA Nod for Quest 3 AI Update
Johnson & Johnson has received US Food and Drug Administration (FDA) clearance for a software update to its Monarch bronchoscopy robot, adding artificial intelligence tools designed to make pre-procedure planning more intuitive for lung cancer diagnosis. Announced on 17 August 2026 as the Quest 3 update, the release marks the Monarch system’s fourth major software launch in the past 18 months and extends J&J’s advanced robotic surgery portfolio.
Monarch assists physicians in performing lung biopsy procedures, providing continuous visualisation to help physicians navigate to suspicious lung nodules and diagnose lung cancer earlier. The Quest 3 update advances planning, navigation and targeting capabilities: newly added AI generates more accurate nodule boundaries automatically during pre-procedural planning, and a new 3D feature improves visual orientation between the joystick controls and patient anatomy. The update also broadens compatibility with commonly used imaging systems and supports enhanced navigation and targeting to address anatomical changes that can occur during the procedure.
Monarch was the first robotic bronchoscopy platform on the market, acquired by J&J through its 2019 acquisition of Auris Health, and competes with Intuitive Surgical’s Ion system and Noah Medical’s Galaxy system. The clearance follows J&J’s July 2026 FDA de novo authorization for its Ottava soft tissue robotic surgery system, which will compete with Intuitive’s da Vinci and Medtronic’s Hugo across the general surgery robotics market. The rapid cadence of J&J’s Monarch software updates, combined with its Ottava launch and Medtronic’s real-time AI enhancements for Hugo, underscores the increasingly rapid pace of AI integration across the surgical robotics category.
The commercial signal is a rapidly expanding intelligent surgery category where iterative AI-driven software refresh cycles are increasingly critical to competitive positioning. Monarch’s fourth software launch in 18 months illustrates the accelerating pace. Expect Intuitive Surgical (Ion), Noah Medical (Galaxy) and other bronchoscopy specialists, alongside Medtronic and other surgical robotics players, to sharpen their own AI update roadmaps through late 2026 and 2027.
