Serbia Study Backs Intraseptal Anaesthesia in Diabetes

A Serbian randomised clinical trial has found that intraseptal anaesthesia (ISA) may offer efficacy advantages over periodontal ligament anaesthesia (PLA) for tooth extraction in patients with controlled Type 2 diabetes. Published in Frontiers in Oral Health on 24 August 2026 and reported by Dental Tribune International on 2 September, the study compared the two techniques on 120 adults, both delivered through a computer-controlled system.

Local anaesthesia in people with Type 2 diabetes requires particular care because of concerns around cardiovascular response to vasoconstrictors and potential susceptibility of nerve fibres to ischaemic injury. ISA and PLA offer alternatives because both can be effective using small amounts of local anaesthetic and vasoconstrictor. The single-centre, single-blind randomised trial assigned participants to ISA or PLA via a computer-controlled anaesthesia delivery system and assessed anaesthetic success, onset, duration and field width alongside cardiovascular parameters.

 

Both techniques produced immediate anaesthesia in successful cases, but ISA generally performed better. For maxillary lateral incisor extraction, ISA achieved 100.0% success versus 93.3% for PLA; for mandibular first premolar extraction, success was 93.3% with ISA versus 70.0% with PLA (this difference was not statistically significant). ISA produced significantly longer soft-tissue anaesthesia (60.5 versus 46.2 minutes for maxillary lateral incisors; 55.8 versus 45.8 minutes for mandibular first premolars) and a significantly wider anaesthetic field for both tooth types. Cardiovascular measurements remained broadly stable for both methods. The authors concluded that both techniques were effective and safe, and that ISA’s longer duration and wider field may make it a preferred option for extraction, subject to multicentre validation.

The commercial signal is a substantial and growing dental patient population where computer-controlled anaesthesia delivery systems and refined injection techniques increasingly matter. Type 2 diabetes affects over 537 million adults globally. Expect Septodont, Dentsply Sirona (STA), Milestone Scientific (Wand STA), 3M, Kerr Dental and Ultradent to sharpen their positioning across the dental anaesthetic and delivery device categories through the rest of 2026 and 2027.
