UK Study Warns of Care Risk Ahead of 2034 Amalgam Ban

A major Newcastle University cost-consequence analysis has found that amalgam restorations outperform composite alternatives across multiple clinical, economic and patient-experience measures, raising serious concerns ahead of the global 2034 amalgam phase-out. Published in the British Dental Journal on 24 July 2026 and reported on 18 August, the study projected that amalgam restorations last longer, generate lower lifetime costs and produce fewer postoperative complications than composite for NHS primary care.

 

The study, led by Dr Oliver Bailey of Newcastle University’s restorative dentistry school, drew on a survey of more than 1,500 UK primary care clinicians, an analysis of public preferences and cost-consequence modelling projecting lifetime outcomes for both materials. Amalgam restorations and restored teeth were projected to last longer than composite alternatives and to generate lower lifetime costs for patients, the NHS and clinicians, with lower estimated risk of postoperative complications. The only outcome quantified in the analysis that favoured composite was appearance. The analysis did not quantify environmental impacts or broader patient and clinician safety considerations.

 

The study flagged wider structural risks: a large majority of UK dentists and therapists expect increased need for both indirect restorations and tooth extractions if amalgam is phased out. The point at which further direct restoration is no longer possible would also be reached sooner with composite than with amalgam, potentially forcing patients into more expensive interventions or extractions. Bailey positioned improved composite training as a clear priority, noting current courses often leave clinicians under-confident in difficult clinical situations, and called for wider NHS dental service reform.

 

The commercial signal is a global amalgam-to-composite transition that will impose real clinical and cost pressures on health systems and patients unless composite materials, indirect restoration workflows and clinician CPD keep pace. Manufacturers and CAD/CAM specialists sit at the centre of this transition. Expect 3M (Filtek), Dentsply Sirona, Ivoclar, VOCO, Kuraray Noritake, GC Corp, Kerr and SDI to sharpen their positioning through 2034.
