CMS Extends TAVR Medicare Coverage in Win for Edwards
The US Centers for Medicare and Medicaid Services (CMS) has finalised a national coverage determination that extends Medicare coverage for transcatheter aortic valve replacement (TAVR) to include severe aortic stenosis patients who are asymptomatic. Announced on 11 September 2026, the update ends the coverage with evidence development requirement for symptomatic patients and is expected to substantially benefit Edwards Lifesciences, the TAVR market leader.
Severe aortic stenosis is a narrowing of the heart valve that blocks blood flow. The updated CMS policy provides Medicare coverage for asymptomatic patients while requiring continued evidence development in this group, and lifts previous evidence development requirements for symptomatic patients. Edwards Lifesciences had petitioned CMS to reconsider its 2019 coverage policy following the FDA’s April 2025 expanded indication approval for the Sapien 3 TAVR platform in asymptomatic patients, based on the EARLY TAVR study which showed asymptomatic patients fared better after TAVR than under clinical surveillance. The global TAVR market is valued at more than $7 billion, per Stifel analysts.
Wall Street analysts positioned the decision as clearly constructive for the overall TAVR market and particularly for Edwards, with BTIG analysts expecting increased TAVR procedure volumes over time. Rival heart valve manufacturer Medtronic welcomed the decision and noted its emphasis on continued evidence generation, deriving 5.5% of revenues from TAVR through its Evolut platform. Edwards confirmed continued confidence in previously communicated 2026 financial guidance. CMS separately said it will review TAVR coverage for patients with aortic regurgitation, with a proposed coverage decision expected by March 2027; JenaValve’s Trilogy became the first TAVR system approved by the FDA for aortic regurgitation in March 2026.
The commercial signal is a substantial expansion of the addressable US TAVR market and reinforcement of Edwards Lifesciences’ dominant category leadership through 2027 and beyond. Aortic regurgitation coverage expansion could open substantial commercial territory. Expect Edwards Lifesciences, Medtronic, Boston Scientific and Abbott to sharpen their TAVR positioning alongside JenaValve for adjacent aortic regurgitation applications through 2027
